Strategic partnering agreement to keep people happy, healthy at home
Health and care organisations across Bradford district and Craven will be working together in partnership to ensure people stay happy, healthy at home.
The Bradford district and Craven strategic partnering agreement (SPA), brings together the NHS, local authority, voluntary and community sector organisations to transform and integrate services for local people. These organisations have worked closely for a long time, this agreement means that they will take decisions together to deliver on the Bradford district and Craven health and wellbeing plan.
One of the key aims is to create sustainable, integrated and high quality services against the challenge of increasing demand. Over the next few years, services will be developed so local people are supported to take action and stay healthy, well and independent throughout their whole life. Prevention and early intervention will be an integral part of the work of the partnership with a greater focus on healthy lifestyles and self-care. Where people need access to care and support, it will be available through proactive joined up health, social care and wellbeing services.
Health and care in Bradford district and Craven is made from two health and care partnerships – the Airedale, Wharfedale and Craven health and care partnership and the Bradford health and care partnership.
